What this template is for
A RACI chart (also called a RACI matrix) maps every task or decision in a project to four roles: Responsible, Accountable, Consulted, and Informed. This template gives you a ready-made grid with six common project activities and five role columns — fill in the R, A, C, and I markers for your own team and you have a clear ownership plan that prevents work from falling through the cracks.
When to use this template
- Kick off a new project by assigning ownership before the first sprint starts.
- Clarify responsibility after a reorg when team boundaries shift.
- Onboard a new stakeholder by showing them where they are consulted or informed.
- Reduce meeting overhead by replacing status calls with a shared RACI reference.
- Document sign-off chains for compliance or audit requirements.
How to use it
- 1List every task, decision, or deliverable in the left column — start with the top five to eight that cause the most confusion.
- 2Write each role or person's name in a header column; keep it to the people who actually touch the work.
- 3For each row, assign exactly one A (Accountable) — the person who signs off and owns the outcome.
- 4Assign R (Responsible) to whoever does the hands-on work; a task can have more than one R if needed.
- 5Mark C (Consulted) for anyone whose input is needed before a decision is made, and I (Informed) for anyone who should be told after.
- 6Review the matrix with your team and fix any rows where accountability is missing or shared.
