What this template is for

A SIPOC diagram is a high-level process map that identifies Suppliers, Inputs, Process, Outputs, and Customers in five columns. It is one of the most useful tools for scoping a process improvement project before diving into detail — you define the boundaries of the process and the key players in one view. Use this template when you need to align a team on what a process actually involves, or as the first step before creating a detailed flowchart or value stream map.

When to use this template

  • Scope a process improvement project by defining the five SIPOC elements before detailed mapping.
  • Onboard a new team member by showing who feeds into and who consumes each process.
  • Prepare for a Six Sigma DMAIC project by documenting the current-state process at a high level.
  • Clarify handoffs between departments by mapping suppliers and customers for each step.

How to use it

  1. 1Start with the Process column — list the 4–7 high-level steps in the workflow.
  2. 2Identify the Outputs of the process — what does each step produce?
  3. 3Identify the Customers who receive each output.
  4. 4Identify the Inputs required for each process step.
  5. 5Identify the Suppliers who provide each input.
  6. 6Review the diagram with stakeholders to confirm the SIPOC is complete and accurate.
